Parkridge Medical Group - Primary Care

For years, Parkridge Medical Group Primary Care has proudly served Chattanooga and surrounding areas as a community provider of quality healthcare. As part of our commitment to our patients and community, we continuously evaluate how to best meet the evolving healthcare needs of those we serve.

After extensive thought and consideration, we have made the difficult decision to close Parkridge Medical Group Primary Care, effective July 31, 2024. We are appreciative of the continued trust that you have shown to us in allowing our practitioners to provide for your medical needs through the years.

If you have an upcoming appointment after July 23, 2024, your appointment will be cancelled. To identify a new provider, please call TriStar Medline at (800) 242-5662. They will provide a referral to a provider who is located close to where you live or work and accepts your insurance.

We will continue to be a custodian of your medical records. To obtain a copy of your medical records, fax the medical release under ‘Patient forms’ to Sharecare at (423) 697-8370.
