New Patients

New patients are asked to arrive 20 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we attempt to notify patients beforehand.

After Hours & Emergencies

If you are experiencing a medical emergency: dial 911; go to Parkridge Medical Center; go to Parkridge East Hospital or your closest emergency department. However, if you need after hours advice for a medical concern, please call and our answering service will assist you.

Prescriptions & Refills

All prescriptions and refill requests should be requested during normal office hours. Please be sure to give us at least 48 hours notice on all refill requests. Please have your pharmacy call the office.


If you are requesting a referral, we ask that you notify our office at least 5 days in advance to allow our staff adequate time to prepare the referral. All referral requests for any new medical condition, requires an appointment with your physician to determine medical necessity.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
