You are the reason we are here and do what we do. We are dedicated to making sure you feel cared for and important. In addition, we are dedicated to providing you with compassionate, high-quality medical care. While at our office or if you have a procedure or surgery and are at the hospital, please do not hesitate to ask us any questions that you may have.

New patients
New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print and complete the patient forms from your physician's office prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
Cancellations or rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we attempt to notify patients beforehand.
After hours & emergencies
If you are experiencing a medical emergency, please dial 911, go to Parkridge Medical Center, Parkridge East Hospital or your closest emergency department. However, if you need after hours advice for a medical concern, please call your physician's office and the answering service will assist you.
Prescriptions & refills
To request a prescription for the first time:
You must be seen in person by one of our physicians.
Appointments can be booked by phone: (423) 756-6623.
To request a prescription refill:
Contact our practice to ask for a refill during business hours (Mon.-Thurs.: 8:00am - 5:00pm and Fri: 8:00am - 2:00pm) , and we will then contact your pharmacy.
If you are out of refills and need to renew your prescription, you will need to schedule an appointment to be seen by one of our physicians.
Billing
We require payment of your deductible, co-payment, and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
Insurance
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan as well as any referral requirements, copays, deductibles or other network requirements from your insurance plan. If you have an HMO or an insurance plan that requires a primary care provider (PCP) referral or authorization, please verify that the referral has been sent to our office prior to your appointment. Lack of referral may result in your appointment being rescheduled.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Referrals
If you are requesting a referral, we ask that you notify our office at least 5 days in advance to allow our staff adequate time to prepare the referral. All referral requests for any new medical condition, requires an appointment with your physician to determine medical necessity.
Medical records
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
